  • ICAK-U.S.A. Doctor Member: $435 (includes dinner pass)
  • ICAK-U.S.A. Doctor Non-Member:  $500 (includes dinner pass)
  • ICAK-U.S.A. In-School Student:  $135; $235 with dinner pass
  • Non ICAK-U.S.A. In-School Student:  $160 (includes ICAK-U.S.A. student membership); $260 with dinner pass
  • CE Credits:  $60

Pre-Conference Workshops, Spouse/Guest Passes, Printed Materials, etc. will be at additional costs. There are special In-School Student pricing for Workshops. Please view the online registration form for details.

 

SPECIAL MEETING ITEMS

  • There will be wireless internet provided in the General Session Room ONLY each day.
  • Electrical outlets will be provided in General Session Room ONLY for attendees to power computers.
  • All meeting materials will be provided on a USB drive.  A printed notebook can be purchased during your registration process for $50.  Please order printed notebooks prior to the meeting as there will be a limited supply for purchase in Dallas.
  • Water stations will be set in the General Session Room and in the Exhibit Hall. Be sure to bring your Vapor bottle from last year.  Only new registrants (those who did not attend in 2011) will be given a Vapor bottle upon check in.  You may purchase a Vapor bottle on site, while supplies last.

Shuttle

The Ritz Carlton Dallas is seven (7) miles from Dallas Love Field airport and eleven (11) miles from DFW airport.
